Bu farkl\u0131l\u0131klar yaln\u0131zca g\u00f6z rengimizi veya boyumuzu belirlemez ayn\u0131 zamanda v\u00fccudumuzun ila\u00e7lara verdi\u011fi yan\u0131t\u0131 da etkileyebilir. Ba\u015fka bir deyi\u015fle ila\u00e7lar\u0131n etkisi yaln\u0131zca ilac\u0131n kendisine de\u011fil, onu kullanan ki\u015finin genetik \u00f6zelliklerine de ba\u011fl\u0131d\u0131r.<\/p>\n<h2><strong>DNA&#8217;m\u0131z ila\u00e7lara verdi\u011fimiz yan\u0131t\u0131 nas\u0131l etkiliyor?<\/strong><\/h2>\n<p>Bir ilac\u0131n etkisini g\u00f6sterebilmesi i\u00e7in \u00f6nce v\u00fccuda al\u0131nmas\u0131, kana kar\u0131\u015fmas\u0131, hedef dokuya ula\u015fmas\u0131, etkisini g\u00f6stermesi ve daha sonra v\u00fccuttan uzakla\u015ft\u0131r\u0131lmas\u0131 gerekir. Bu s\u00fcre\u00e7lerin tamam\u0131 \u00e7ok say\u0131da protein ve enzim taraf\u0131ndan y\u00f6netilir. Bu proteinleri ve enzimleri \u00fcreten talimatlar ise genlerimizde bulunur. E\u011fer bu genlerde k\u00fc\u00e7\u00fck farkl\u0131l\u0131klar varsa ila\u00e7lar\u0131n v\u00fccuttaki yolculu\u011fu da ki\u015fiden ki\u015fiye g\u00f6re de\u011fi\u015febilir. \u00d6rne\u011fin baz\u0131 insanlar\u0131n b\u00fcnyesi bir ilac\u0131 \u00e7ok h\u0131zl\u0131 par\u00e7alar. Bu ki\u015filerde ila\u00e7 v\u00fccuttan k\u0131sa s\u00fcrede uzakla\u015ft\u0131r\u0131ld\u0131\u011f\u0131 i\u00e7in beklenen tedavi etkisi ortaya \u00e7\u0131kmayabilir. Buna kar\u015f\u0131l\u0131k baz\u0131 ki\u015filerde ayn\u0131 ila\u00e7 \u00e7ok daha yava\u015f par\u00e7alan\u0131r. B\u00f6yle durumlarda ila\u00e7 uzun s\u00fcre v\u00fccutta kald\u0131\u011f\u0131 i\u00e7in yan etki g\u00f6r\u00fclme olas\u0131l\u0131\u011f\u0131 artabilir. Bu nedenle herkese ayn\u0131 ilac\u0131n ayn\u0131 dozda verilmesi her zaman en do\u011fru yakla\u015f\u0131m olmayabilir. Farmakogenetik tam da bu farkl\u0131l\u0131klar\u0131 ara\u015ft\u0131ran bilim dal\u0131d\u0131r. Ama\u00e7, her birey i\u00e7in en uygun ilac\u0131 ve en do\u011fru dozu belirleyerek tedavinin hem daha g\u00fcvenli hem de daha ba\u015far\u0131l\u0131 olmas\u0131n\u0131 sa\u011flamakt\u0131r.<\/p>\n<h2><strong>Peki genetik farkl\u0131l\u0131klar nas\u0131l ortaya \u00e7\u0131kar?<\/strong><\/h2>\n<p>\u0130nsanlar aras\u0131nda en s\u0131k g\u00f6r\u00fclen genetik de\u011fi\u015fikliklerden b<strong>iri tek n\u00fckleotit polimorfizmi (Single Nucleotide Polymorphism, SNP) <\/strong>olarak adland\u0131r\u0131l\u0131r. Basit\u00e7e ifade etmek gerekirse DNA dizisindeki tek bir n\u00fckleotidin farkl\u0131 olmas\u0131 anlam\u0131na gelir. Bu k\u00fc\u00e7\u00fck de\u011fi\u015fiklik bazen bir enzimin \u00e7al\u0131\u015fma h\u0131z\u0131n\u0131 de\u011fi\u015ftirebilir. Ya\u015f, cinsiyet, beslenme al\u0131\u015fkanl\u0131klar\u0131, kullan\u0131lan di\u011fer ila\u00e7lar, sigara ve alkol kullan\u0131m\u0131, karaci\u011fer ve b\u00f6brek fonksiyonlar\u0131 gibi bir\u00e7ok \u00e7evresel ve biyolojik etken de tedavi ba\u015far\u0131s\u0131n\u0131 etkiler. Bununla birlikte yap\u0131lan ara\u015ft\u0131rmalar, genetik farkl\u0131l\u0131klar\u0131n ila\u00e7 yan\u0131t\u0131ndaki bireysel de\u011fi\u015fkenli\u011fin \u00f6nemli bir b\u00f6l\u00fcm\u00fcnden sorumlu oldu\u011funu g\u00f6steriyor. Bu nedenle g\u00fcn\u00fcm\u00fczde tedaviler planlan\u0131rken hastan\u0131n genetik \u00f6zellikleri de giderek daha fazla dikkate al\u0131n\u0131yor.<\/p>\n<p>Farmakogenetik \u00f6zellikle kanser tedavisinde dikkat \u00e7ekici uygulama alanlar\u0131 bar\u0131nd\u0131r\u0131yor. Baz\u0131 kanser ila\u00e7lar\u0131 uygulanmadan \u00f6nce hastan\u0131n t\u00fcm\u00f6r\u00fcnde ya da genetik yap\u0131s\u0131nda belirli de\u011fi\u015fikliklerin bulunup bulunmad\u0131\u011f\u0131 ara\u015ft\u0131r\u0131l\u0131yor. B\u00f6ylece hangi hastalar\u0131n belirli bir tedaviden yarar g\u00f6rme olas\u0131l\u0131\u011f\u0131n\u0131n daha y\u00fcksek oldu\u011fu \u00f6ng\u00f6r\u00fclebiliyor. Benzer \u015fekilde baz\u0131 kan suland\u0131r\u0131c\u0131lar, antidepresanlar ve a\u011fr\u0131 kesiciler i\u00e7in de genetik testler sayesinde uygun ila\u00e7 se\u00e7imi veya doz ayarlamas\u0131 yap\u0131labiliyor. B\u00f6ylece gereksiz ila\u00e7 kullan\u0131m\u0131n\u0131n ve istenmeyen yan etkilerin azalt\u0131lmas\u0131 hedefleniyor.<\/p>\n<p>Bu yakla\u015f\u0131m bizi ki\u015fiselle\u015ftirilmi\u015f t\u0131p anlay\u0131\u015f\u0131na g\u00f6t\u00fcr\u00fcr. Ki\u015fiselle\u015ftirilmi\u015f t\u0131pta ama\u00e7, herkese ayn\u0131 tedaviyi uygulamak yerine her bireyin genetik yap\u0131s\u0131n\u0131, ya\u015fam tarz\u0131n\u0131 ve sa\u011fl\u0131k durumunu birlikte de\u011ferlendirerek en uygun tedavi plan\u0131n\u0131 olu\u015fturmakt\u0131r. Yak\u0131n gelecekte bir ilaca ba\u015flamadan \u00f6nce ki\u015finin genetik \u00f6zelliklerinin de\u011ferlendirilmesi, kan tahlili yapt\u0131rmak kadar rutin bir uygulama olabilir. B\u00f6ylece deneme yan\u0131lma y\u00f6ntemiyle ila\u00e7 se\u00e7mek yerine ki\u015finin genetik yap\u0131s\u0131na en uygun tedavi ilk a\u015famada planlanabilir.<\/p>\n<p>K\u0131sacas\u0131 ayn\u0131 ilac\u0131n herkeste ayn\u0131 etkiyi g\u00f6stermemesinin \u00f6nemli nedenlerinden biri genetik farkl\u0131l\u0131klar\u0131m\u0131zd\u0131r. Farmakogenetik, bu farkl\u0131l\u0131klar\u0131 anlamam\u0131z\u0131 sa\u011flayarak daha etkili ve daha g\u00fcvenli tedavilere kap\u0131 aral\u0131yor. Gelece\u011fin t\u0131bb\u0131nda ila\u00e7lar yaln\u0131zca hastal\u0131\u011fa g\u00f6re de\u011fil ayn\u0131 zamanda hastan\u0131n genlerine g\u00f6re de se\u00e7ilecek gibi g\u00f6r\u00fcn\u00fcyor.
